My training had prepared me for the practicalities of volunteering, but it had not fully prepared me for the emotional weight of a patient whose physical problems could no longer be fixed. Over the next few weeks, my visits with “Melanie” transformed my understanding of what it means to truly be empathetic and care.

Our conversations were often surface-level, talking about the weather, or her meals at the home, but they soon deepened into a relationship that has left a mark on me. We spent many of our visits discussing her husband’s passing, and followed each conversation with a short piano piece in an attempt to brighten her day. Most days, I was not greeted with a welcoming smile, but with much persistence, I was able to visit “Melanie.” Watching the PBS Frontline documentary Being Mortal fundamentally shifted the lens through which I viewed “Melanie’s” care. Dr. Atul Gawande’s exploration of end-of-life care highlights a critical blind spot in modern medicine: our relentless focus on extending life often overshadows the patient’s quality of life. Gawande emphasizes the importance of asking patients what a “good day” looks like to them. Inspired by this, I asked “Melanie” that very question on our first visit of the two of us. With much hesitation, “Melanie” told me she enjoyed music, sitting quietly, and watching T.V.

This realization marked a significant period of personal growth for me. I had to unlearn the instinct to constantly search for an answer like beginning a conversation, and instead learn to sit comfortably in the reality of terminal decline. I learned that just being in “Melanie’s” presence is often just as powerful as a prescription medicine that could have healed her. In the past, I might have viewed “Melanie’s” transition to hospice as a failure of her physician. Instead, I began to see it as a courageous prioritization of what mattered most to her: comfort and autonomy.

This experience has profoundly shaped my sense of vocation. Before volunteering, my motivation for medical school was rooted almost entirely in the science of saving lives, and the curiosity of the human body. Now, I understand that my true joy for medicine must equally encompass guiding patients through the end of life, no matter the drawbacks I may have. Through this process I have learned that medicine is not just about defying mortality, but rather about navigating the end of life with dignity. I now realize that even when medical intervention reaches its limits, a physician’s capacity to provide compassionate, patient-centered care does not truly have an end. When I describe the value of this hospice experience in my medical school application, I will emphasize that it provided me with an irreplaceable education in humanism, and patient-centered compassionate care. This program bridges my gap between the idea of how clinical knowledge and empathetic practice are equally important, ensuring that I will enter medical school with the ability to effectively care for others.

When I first met “Melanie,” I was an aspiring physician who viewed medicine primarily as a series of problems to be solved, and had never thought about the emotional aspect as a physician.
